How do you migrate Single-Collateral SAI to Multi-Collateral DAI?

On November 18, 2019, MakerDAO launched Multi-Collateral DAI and renamed Single-Collateral DAI to SAI.

All SAI holders are required to migrate into DAI at a 1:1 value.
